The National Health Mission (NHM), Uttar Pradesh invites online applications for 04 Month Certificate in Community Health for Nurses (CCHN) Training for the 2022-23 session. This course is a part of initiative
Under the Ayushman Bharat Scheme, GoUP envisioned to strengthen Sub-Health Centers as Health and
Wellness Centers (HWCs) for improved implementation of Public Health Programs and to enable
comprehensive primary healthcare service delivery, including disease prevention and health promotion. Candidates who successfully complete this course and pass the final examination will be posted as
Community Health Officer (CHO) at Sub-Health Centers level HWCs on contract basis. The CHOs has to work
in coordination with primary healthcare team.

EXAM PATTERN
- The Computer based Online Test will be held for 2 hours.
- The test consists of two sections, Section-I (80 Marks) and Section-II (20 Marks)
- Section I:Â Professional Knowledge (Discipline related).
- Section II:Â General Aptitude, Reasoning, General Awareness and Basic Computer knowledge with 100 multiple choice questions (MCQ).
- Each question will carry 1 mark and there is no negative marking for wrong answer.
- No marks will be awarded for un-attempted questions.
- The question paper shall be set in bilingual language i.e. English and Hindi only.
- However, in case of any inconsistency in Hindi version, the English version of the question shall be valid & final.

- Click Opportunities -> Apply Online Button next to Recruitment of Community Health Officer (CHO) under National Health Mission, Uttar Pradesh.
- Online registration will remain open till 09/08/2022.
- Register using a valid email-id and mobile number.
- Login and apply using the generated user ID and Password.
- Candidates fill in the online form with all the relevant correct details.
- Submit the Online Application.
- Take a printout of the finally submitted Online Application.
